C-Lys develops information for patients. We find it very important that this information is clear and understandable! Accurate information provides a lot of advantages. Together with the consultation of the healthcare provider, it can help patients to make informed choices in their care process. Informed patients follow their treatment better and more in consultation with their healthcare provider. 

By the means of this survey, C-Lys wants to learn more about the experiences and opinions of people with implants (e.g. pacemakers, hip prosthesis, breast reconstruction with implants, ...). The answers you provide will not be linked to you since we don't collect your contact details. As a patient, have you been informed sufficiently about your implant? From whom and how did you receive the information?
